Your sponsor will need to contact the church where they are a registered parishioner and complete their requirements to obtain a Statement of Faith (Sponsor Form)

If your sponsor is a parishioner of St. Katharine Drexel Mission or St. Stephen the Martyr, please have them stop by during office hours to complete and sign the form with one of our staff as witness and add the church seal to the document.
